Terms and Conditions

The terms and conditions of this web site are simply that in order to register, one must be a licensed M.D., D.O., non-physician pain practitioners (nurses, physicians assistants, and or other allied health personnel or trainee including those executing fellowship.
The fee for membership is merely $5 a year. This token fee covers the annual meetings.
To become a member, simply complete our
Membership Form online or by downloading a Word document or PDF file to print and fax or mail back to our office
